Goals
Our program goal is to prevent cardiovascular disease and other chronic diseases such as Type 2 diabetes, stroke and some forms of cancer. We achieve this goal through the development of collaborative partnerships across Ontario.
Unique Community Partnership Model
The OHHP - Taking Action for Healthy Living is a leader in coordinating and mobilizing community partners to deliver programs that result in behaviour change and healthy lifestyles for all Ontarians.
This model allows for community ownership of healthy living programs because local communities decide where to allocate provincial funding and other resources in order to achieve local and provincial goals.
In 2005, over 2,300 community partnerships were involved in delivering over 700 initiatives targeting healthy eating, physical activity and tobacco. The value of in-kind contributions exceeded $16 million in 2005 alone. This equals a 4:1 ratio of in-kind contribution to provincial dollars.
